Laminitis vs. Founder — Understanding the Difference
These two terms are often used interchangeably but they describe different stages of the same disease process.
🔬 The Critical Distinction
Laminitis is the inflammation of the sensitive laminae — the interlocking tissue that bonds the hoof wall to the coffin bone (pedal bone). The laminae act like Velcro holding the two structures together. When they become inflamed, their microscopic interlocking structures begin to fail, and the bond weakens. Laminitis is the process — the active inflammatory event.
Founder is what happens when laminitis has been severe enough that the laminae fail structurally — the coffin bone rotates downward (rotation) or sinks vertically inside the hoof capsule (sinking). Founder is the structural consequence of severe or inadequately treated laminitis. A horse can have laminitis without foundering; a horse that has foundered has experienced laminitis severe enough to cause permanent mechanical change.
In everyday conversation, most horse owners use "founder" loosely to mean any laminitis episode — this is clinically inaccurate but practically common. What matters is recognizing the signs and responding immediately regardless of which word you use.
What Causes Laminitis?
Laminitis is not a single disease — it is the final common pathway of multiple different triggers. Knowing your horse's risk factors is the key to prevention.
Pasture Laminitis (EMS / Insulin Dysregulation)
The leading cause. Horses with Equine Metabolic Syndrome (EMS) have chronically elevated insulin, which directly damages lamellar cells. Rich spring and fall grass (high in non-structural carbohydrates) triggers an insulin spike that can cause laminitis within hours in susceptible horses. Ponies, Morgans, Warmbloods, and easy keepers are highest risk.
Grain Overload (Carbohydrate Overload)
A horse that breaks into the feed room and consumes a large quantity of grain, or is fed a dramatically increased ration, can develop acute laminitis within 24–48 hours. The carbohydrate load overwhelms the small intestine, passes to the hindgut, causes rapid fermentation, dysbiosis, and a cascade of inflammatory signals that reach the lamellar tissue.
PPID (Cushing's Disease)
Pituitary Pars Intermedia Dysfunction — the most common endocrine disease in horses over 15 years old — causes chronically elevated ACTH and cortisol, which drive insulin resistance. PPID horses develop recurrent laminitis year-round, not just seasonally. Any older horse with a long, wavy, or patchy coat that develops laminitis should be tested for PPID immediately.
Supporting-Limb Laminitis
When a horse bears excessive weight on one limb for an extended period — typically because the opposite limb is injured and non-weight-bearing — the overloaded hoof can develop laminitis from vascular compromise. This is a serious complication of major limb injuries and is monitored carefully during recovery from fractures, tendon injuries, and joint infections.
Sepsis / Systemic Illness
Severe systemic illness — retained placenta, colitis, pleuropneumonia, severe colic — can trigger laminitis through endotoxemia and inflammatory cascade. Any hospitalized horse with a serious systemic illness is considered at laminitis risk and monitored accordingly, often with ice therapy applied prophylactically to the feet.
Steroid-Induced Laminitis
Corticosteroids — particularly injectable dexamethasone or triamcinolone — can trigger laminitis in susceptible horses, especially those with underlying EMS or PPID. The risk is real but often overstated; appropriate use of steroids in healthy horses rarely causes laminitis. However, horses with known metabolic disease require extra caution and alternatives where possible.
Recognizing Laminitis — Signs by Stage
Early recognition is everything. The difference between a horse that recovers fully and one that founders often comes down to how quickly treatment begins.
🔍 Laminitis Stages — What You Will See
Stage 1
The horse is reluctant to move or walk. Classic laminitic stance: weight shifted back onto the heels, front feet stretched forward, hindquarters tucked under. One or both front feet feel hot to the touch. A strong, bounding digital pulse is palpable at the back of the fetlock. The horse may appear anxious, sweat, or show signs of pain. Radiographs at this stage may show normal coffin bone position — the laminae are inflamed but have not yet failed mechanically. This is the window in which aggressive treatment can prevent founder.
Stage 2
Clinical signs of acute laminitis are present but may fluctuate. Radiographs may begin to show early coffin bone rotation — the tip of the bone tilting downward relative to the hoof wall. The horse may appear to improve and worsen repeatedly. This is a dangerous stage — a horse that appears to be "getting better" may simply be moving from acute to subacute, not recovering. Continue strict stall rest and veterinary guidance; do not interpret reduced lameness as permission to return to normal management.
Stage 3
The coffin bone has rotated or sunk. Growth rings visible on the hoof wall — wider at the heels than the toe (a classic founder sign). The white line may be stretched and separated (seedy toe). The sole may appear flattened or convex ("dropped sole") as the rotated coffin bone presses downward. The horse has persistent low-grade lameness or is chronically foot-sore. This stage requires long-term collaborative management between farrier and vet, and some horses never return to full athletic work.
Stage 4
In the most severe cases, the tip of the rotated coffin bone penetrates the sole of the hoof — visible as a dark discoloration or actual protrusion at the toe. This is a catastrophic outcome requiring intensive veterinary and farrier intervention. Prognosis for return to soundness is guarded to poor depending on the degree of penetration. Euthanasia may be the most humane option in severe cases. This outcome is almost entirely preventable with early treatment of acute laminitis.
The digital pulse is felt at the back of the fetlock, where the digital arteries run on either side. In a healthy horse the pulse is barely detectable. In laminitis, it becomes strong and bounding — like pressing your fingertip against a garden hose under pressure. Practice feeling for it now on a healthy horse so you know the difference when it matters. A bounding digital pulse with any reluctance to move = call your vet.

Treatment — Three Roles, Three Approaches
Laminitis requires the tightest farrier-vet-owner coordination of any hoof condition. No single role can manage this alone.
The Farrier's Role — Mechanical Support, Therapeutic Shoeing, Long-Term Rehabilitation
The farrier cannot stop laminitis — that is the vet's domain. But the farrier controls the mechanical environment of the hoof, and in laminitis that environment is everything. The right support applied at the right time dramatically changes outcomes. The wrong trim or shoe can accelerate rotation and destroy a horse that might otherwise have recovered.
In the acute phase (first 48–72 hours), the farrier's first and most important contribution is often providing immediate mechanical support without disturbing the hoof. The inflamed laminae are structurally weakened — aggressive trimming or manipulation can accelerate rotation. Immediate measures include:
- Deep sand or dirt bedding — the horse sinks in, providing conforming ground pressure that supports the entire sole rather than just the hoof wall
- Foam padding — a thick block of medium-density foam (Styrofoam or commercial laminitis foam pads) placed under the feet provides whole-sole support and reduces toe leverage
- Frog support — a frog support pad or rolled bandage applied to take pressure off the rotated bone tip
- Do not apply a traditional shoe with nail holes in acute laminitis — any rotation makes nail placement unpredictable and potentially dangerous
Once the vet has taken lateral radiographs showing the exact position of the coffin bone, the farrier uses those images to guide the trim. The goal is to realign the bottom of the coffin bone parallel to the ground — restoring the relationship between bone and ground surface that laminitis disrupts. This typically means:
- Aggressive toe reduction — removing excess toe that extends in front of the coffin bone tip, reducing the lever arm pulling the bone forward and down
- Heel lowering (if heels are high) — long heels increase the rotational force on the coffin bone; the farrier lowers heels to reduce this load, working carefully to avoid over-stressing the already-compromised deep digital flexor tendon
- Sole preservation — the sole is never thinned in a laminitic horse; it provides the only protection against coffin bone penetration
Once acute pain is managed and the trim is established, the farrier selects a therapeutic shoeing strategy. Common approaches include:
- Egg bar shoe with heel elevation — extends the shoe behind the heels for added support; the slight heel elevation reduces tension on the deep digital flexor tendon, which attaches to the back of the coffin bone and pulls it into rotation
- Heart bar shoe — includes a bar that contacts and supports the frog directly, transferring weight from the hoof wall (compromised) to the frog; requires precise fit — too much frog pressure is worse than none
- Wooden or aluminum clog (Natural Balance style) — breaks over at the widest part of the foot, dramatically reducing toe leverage
- Glue-on shoes — applied without nails, preserving more sole and avoiding nail trauma to weakened laminae; particularly useful when hoof wall integrity is poor
- Deep pour pads — pour-in polyurethane padding fills the entire solar surface, providing conforming whole-sole support; often combined with bar shoes
In chronic founder cases with established rotation, the farrier undertakes a long-term rehabilitation program — often 12–24 months — working through successive trim cycles to gradually restore hoof geometry as new wall grows. Key milestones include:
- Monitoring the "lamellar wedge" — the stretched, low-quality horn between the hoof wall and rotated coffin bone — and managing its gradual resolution
- Addressing seedy toe and WLD that commonly develop in the stretched white line
- Adjusting shoeing at each 4–5 week visit as hoof geometry changes with new growth
- Working from radiographs taken every 3–4 months to confirm the coffin bone's relationship to the ground and hoof wall is improving
Deep Pour Pads (Vettec Equi-Pak)
Polyurethane pour-in pad that conforms to the entire solar surface. Provides whole-sole support, protects the sole from ground trauma, and cushions the coffin bone tip. Standard of care in acute laminitis.
Heart Bar Shoe
Steel or aluminum shoe with a frog-contact bar. Transfers weight from compromised hoof wall laminae to the frog. Requires precise fit — farrier must balance frog pressure carefully against rotation degree.
Glue-On Shoes
Applied with adhesive rather than nails. Preserves weakened hoof wall. Available in aluminum, composite, and thermoplastic materials. Ideal for horses with poor wall quality post-laminitis.
Laminitis Foam / Styrofoam Pads
Medium-density foam blocks placed under the feet in acute cases before shoeing is possible. The horse sinks in, receiving conforming whole-sole support. Inexpensive, immediately available, genuinely effective first aid.
Trimming a laminitic horse without current radiographs is guessing. The coffin bone's position relative to the hoof wall and ground surface cannot be accurately assessed by external appearance alone. A trim that looks correct from the outside can be catastrophically wrong internally. Every laminitis case requires radiographs before significant trimming. This is non-negotiable in any serious laminitis case.
The Veterinarian's Role — Emergency Response, Diagnosis, Medical Management
The vet leads the laminitis response. Pain management, identifying and removing the trigger, radiographic monitoring of the coffin bone, and treating any underlying metabolic disease are all veterinary territory. The farrier cannot do this work; the vet directs the overall treatment plan.
The first and most important vet action is identifying and eliminating the cause. This happens in parallel with pain management:
- Pasture laminitis — immediately remove from grass, restrict to dry lot or stall, remove all hay with high NSC content
- Grain overload — administer activated charcoal or mineral oil to slow carbohydrate absorption; consider laxative treatment
- Sepsis-related — treat the primary systemic illness aggressively; the laminitis is a secondary complication
- Supporting-limb — provide the best possible pain management for the primary limb injury and support the other limb proactively
Effective pain control is both humane and therapeutic — a horse in extreme pain will not stand properly on the affected feet, which worsens mechanical forces on the laminae. Standard pain management includes:
- Phenylbutazone (Bute) — NSAID; first-line pain control; also has anti-inflammatory effects in the laminae; dose and duration directed by vet
- Flunixin meglumine (Banamine) — NSAID; particularly useful when endotoxemia is involved (septic laminitis)
- Acepromazine — a tranquilizer historically used to improve digital blood flow through vasodilation; still used by some vets in acute cases
- Opioids — butorphanol for severe acute pain; short-term use in hospital settings
Continuous digital cryotherapy (icing the feet and lower limbs) in the first 24–72 hours of acute laminitis is one of the most effective treatments available — reducing inflammation in the laminae, slowing enzymatic destruction of lamellar tissue, and improving outcomes. The feet are immersed in ice-water slurry or fitted with ice boots continuously during the acute phase. This is now a standard of care recommendation in acute laminitis, particularly in grain overload and sepsis-related cases.
Lateral radiographs of the affected feet are taken at the initial emergency visit to establish baseline coffin bone position, and repeated at intervals (48–72 hours in acute cases; every 4–6 weeks in chronic cases) to monitor for rotation or sinking. The vet measures the degree of rotation in degrees (mild: under 5.5°; severe: over 11°) and uses this to direct trimming and shoeing. Radiographs also guide decisions about prognosis — severe rotation or sinking changes the outlook significantly.
After the acute crisis, the vet investigates the underlying cause to prevent recurrence:
- ACTH test (seasonal) — rules out PPID; elevated in autumn even in normal horses, so timing matters
- Insulin and glucose — diagnoses EMS and insulin dysregulation; the oral sugar test (OST) is more sensitive than resting insulin alone
- Pergolide (Prascend) — medication for PPID; reduces ACTH and associated laminitis risk significantly
- Dietary management — low NSC hay (tested, under 10% NSC), no grain, no lush pasture for EMS horses; grazing muzzle during unavoidable turnout
Phenylbutazone (Bute)
NSAID pain control and anti-inflammatory. First-line medication in acute laminitis. Dose and duration determined by vet — never administer without guidance; long-term use risks gastric and kidney complications.
Pergolide (Prascend)
Dopamine agonist for PPID management. Dramatically reduces ACTH levels and associated laminitis risk in Cushingoid horses. Daily oral medication; most horses require lifelong treatment.
Flunixin Meglumine (Banamine)
NSAID particularly effective when endotoxemia is involved. Used in sepsis-related laminitis and colitis-associated cases where systemic inflammatory load is high.
Continuous Cryotherapy
Ice-water slurry immersion or commercial ice boots applied continuously for 24–72 hours in acute cases. One of the most effective interventions available. Requires commitment — the benefit requires uninterrupted cold application.
The Horse Owner's Role — Recognition, First Aid, Daily Management
In laminitis, the owner's role shifts with the phase. In the acute phase, your job is fast recognition and immediate action — call the vet, confine the horse, provide deep bedding, and begin ice if available. In the recovery and chronic phases, your job becomes the daily discipline of diet management, environment management, and monitoring that determines whether the horse stays sound long-term.
Know the signs and respond the moment you see them:
- Laminitic stance (weight shifted back, front feet stretched out) — call vet immediately
- Horse reluctant to walk or turn — do not force movement; confine to stall
- Hot hooves and bounding digital pulse — call vet immediately
- Horse that got into the grain room — this is an emergency even before symptoms appear; call vet now
- Do not give bute without vet instruction — you may mask symptoms that the vet needs to assess
While waiting for the vet, confine the horse to a deeply bedded stall — 6–8 inches of shavings, sand, or pea gravel. Do not use mats alone — hard flat surfaces increase pain and worsen mechanical forces on the rotated bone. The horse should be able to sink their feet in. Avoid moving the horse unless absolutely necessary — every step on hard ground risks further lamellar tearing.
If the vet is on their way and you have ice and buckets available, begin digital cryotherapy immediately — this is genuinely beneficial and will not harm a horse. Fill buckets with ice water and stand the horse's front feet in them. Commercial ice boots are ideal but buckets work. Continuous icing for 24–72 hours is the target; any icing before the vet arrives is better than none.
For EMS and PPID horses — which is the majority of laminitis cases — the owner's long-term daily management decisions are the most important factor in preventing recurrence:
- Test your hay — target under 10% NSC (non-structural carbohydrates); soaking hay for 30–60 minutes reduces NSC by up to 30%
- No lush grass — remove high-risk horses from pasture or use a grazing muzzle; avoid turnout during peak sugar hours (afternoon, after frost, during rapid growth)
- No grain — feed a ration balancer or low-NSC complete feed instead of traditional sweet feeds or whole grains
- Weigh feed, not volume — obesity is the single most correctable laminitis risk factor; body condition score 4–5 of 9 is the target
- Daily pergolide administration — for PPID horses; establish a routine and never miss a dose
During recovery, monitor and record daily:
- Digital pulse intensity — improving, same, or stronger?
- Hoof temperature — cooling or still hot?
- Willingness to move and bear weight — better or worse?
- Appetite and general demeanor
- Any changes in the appearance of the hoof — new rings, sole bulging, white line changes
A properly fitted grazing muzzle reduces grass intake by 70–80% while allowing turnout and movement — both essential for metabolic horses. Resistance from horses (and owners) to using muzzles is common. The alternative — keeping a laminitis-prone horse off pasture entirely — reduces exercise and increases the isolation and stress that also worsen metabolic disease. A muzzle is the better tool. Introduce it gradually and check the fit daily.
Prevention — The Best Laminitis Treatment Is the One You Never Need
The majority of laminitis cases are preventable. Most are driven by diet and metabolic disease — conditions fully within an owner's control to manage.
Maintain Healthy Body Weight
Obesity is the single most important modifiable laminitis risk factor. An overweight horse has chronically elevated insulin regardless of diet. Target body condition score 4–5 of 9. Reduce feed before reducing exercise — a fat horse should not be worked into shape while overweight.
Manage Pasture Turnout Carefully
Restrict or eliminate pasture access for high-risk horses. Grass NSC peaks in the afternoon and after frost. Use a grazing muzzle, limit turnout to morning hours, or use a dirt paddock during high-risk seasons (spring and autumn flush growth).
Test for PPID and EMS
Any horse over 15 with a history of laminitis, regional fat deposits, or a shaggy coat should be tested annually for PPID. Any easy keeper or overweight horse should have an insulin/glucose panel. Early diagnosis and treatment with pergolide dramatically reduces recurrence.
Secure the Feed Room
Grain overload laminitis is entirely preventable. Use horse-proof latches on all grain storage. Never leave bags of feed accessible. One unsupervised hour with a grain bin has ended horses' careers and lives. This is a zero-tolerance management standard.
Regular Farrier Visits
Long toes create the mechanical conditions that worsen and accelerate rotation in a laminitic episode. Regular trimming on a 5–6 week schedule for high-risk horses keeps toe length in check and provides your farrier the opportunity to catch early white line changes that signal developing problems.
Prophylactic Icing During Systemic Illness
If your horse develops colitis, a serious infection, or any condition your vet identifies as laminitis-risk, begin digital cryotherapy immediately — before any laminitis signs appear. Icing is safe, inexpensive, and genuinely effective as prophylaxis. Do not wait for the bounding pulse to start the ice.
